Student Incidents

All accidents and injuries sustained by Iowa State University students while in academic classes or events sponsored by the university must be reported to Risk Management by the student and a university representative.  Reporting requirements are found in the Accidents and Injuries – Students policy. 

Emergency Medical Assistance

Dial 911 to reach emergency assistance to send an ambulance in life threatening situations (respiratory difficulty, chest pain, profuse bleeding and unresponsiveness).

Non-urgent Medical Services

The Thielen Student Health Center is available on campus to provide non-urgent medical services for students, but does not have emergency room services. If the injured person is willing and able to walk to Thielen Student Heath Center accompanied by another student or faculty/staff member for treatment they may do so.  However, calling 911 may be the best choice when the situation is beyond basic first aid, so that trained first responders can make decisions to address the injury that is presented.

Incidents involving student hospitalization or death

  • Within 8 hours, report the incident to the Office of Risk Management, 515-294-7711 and the Department of Public Safety (DPS), 515-294-4428.
  • Complete the online ISU Incident Form
    • An ISU Net-Id is required to use the online ISU Incident Form. If you do not have an ISU Net-ID please contact us at 515-294-7711 or claims@iastate.edu.
  • Additional assistance is also available from the Dean of Students Office at 515-294-1020.

Incidents not involving hospitalization or death:

  • Complete the online ISU Incident Form
    • An ISU Net-Id is required to use the online ISU Incident Form. If you do not have an ISU Net-ID please contact us at 515-294-7711 or claims@iastate.edu.

Questions about Fault or Liability

Questions regarding liability should be directed to the Office of Risk Management. If appropriate, any reimbursements by the University will be coordinated through the Office of Risk Management.
