Property Claims

To report claims involving property loss and/or damages to University owned property, click here for the ISU Property Loss/Damage Notice.

Theft or Vandalism Claims

If the loss is due to theft or vandalism and occurred on University property, immediately contact the Department of Public Safety (515) 294-4428. If it occurred elsewhere, contact the police department for that jurisdiction. The Property Loss/Damage Notice will need to be completed and returned to Risk Management also.

General Liability Claims

To report claims involving injuries or damage to third parties such as students or visitors, click here for General Liability Loss Notice.

Student Accident Reports

All accidents and injuries sustained by Iowa State University students while in academic classes or events sponsored by the university must be reported using the Student Accident Report Form. The Student Accident Report should be completed by the student and a university representative and sent to Risk Management at 3618 ASB. Questions regarding university liability should be directed to Risk Management at (515) 294-7711.
