General Liability Claims
Departments are responsible for reporting all incidents such as accidents, injuries or property damages affecting members of the general public, or visitors while on campus. Student Accident Reporting
All accidents and injuries sustained by Iowa State University students while in academic classes or events sponsored by the university must be reported using the Student Accident Report Form. Forms may be downloaded from this site or requested from the Office of Risk Management (515-294-7711).
In addition to completing the Student Accident Report Form, incidents that involve hospitalization or death of a student must be reported by the department to Risk Management (515-294-7711) within 8 hours. Incidents occurring after regular business hours should be reported to the Department of Public Safety (DPS) at 515-294-4428. Additional assistance is also available from the Office of the Dean of Students (515-294-1020).
The Student Accident Report should be completed by the student and a university representative and sent to Risk Management at 3618 ASB. A copy of the form and any related documentation pertaining to the incident should also be kept confidentially in the department for future reference. This documentation will assist the University in taking appropriate steps or corrective measures to eliminate hazards that may be connected with University activities.
